What is your financial policy?
Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money-order, MasterCard or Visa.

What insurance do you accept?
We participate with traditional Medicare and several PPO organizations. Please call us to obtain a current list of the participating carriers.

What are your office hours?
Monday 8 a.m. — 4 p.m.
Tuesday 8 a.m. — 5 p.m.
Wednesday 9 a.m. — 5:30 p.m.
Thursday 8 a.m. — 4 p.m.
Friday 9 a.m. — 4 p.m.

Please note hours may vary and it is best to call prior to arrival.

We have coverage 7 days a week 24 hours a day for emergencies.

What should I bring to my appointment?
Be sure to bring your insurance card, ID card or Driver's License, referral if needed, and workers compensation authorization if needed. Also bring any previous doctor notes, x-rays, MRI or CT Films and/or surgery pictures.

Please bring your current and past athletic shoes and shoe inserts/orthotics.

Can I complete my paperwork ahead of time?
Our patient information forms may be downloaded from this website, completed and brought to your initial appointment.
